• Unit-dose (pouch) medication packaging customized for seniors in care—improving adherence and reducing confusion.
• Quarterly medication reviews for residents: a structured evaluation of every resident’s medications with the goal of optimising therapy, resolving issues, and reducing risk of adverse events (falls, interactions, cognitive effects).
• Regular “chemical restraint” reviews for seniors’ homes—evaluating psychotropic or sedative‐type medications and supporting de-prescribing when appropriate.
